An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature’s controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature. An incorrect air bag may have been installed when the steering yoke or round steering wheel was replaced with a different style of yoke or steering wheel. The front-row seat belts may not be connected properly to the pretensioner anchors, which can cause the seat belts to detach. 1970-01-01	STEERING:AUTOMATED/ADAPTIVE STEERING	Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver’s adjustment of the vehicle’s speed to exceed posted speed limits. The electronic power assist steering (EPAS) system may experience a loss of power steering assist when driving on rough roads or after hitting a pothole. 2022-01-11	VISIBILITY:REARVIEW MIRRORS/DEVICES:EXTERIOR	Tesla, Inc. (Tesla) is recalling certain 2021 Model S exterior side rearview mirrors, part numbers: 1622232-00-B and 1622233-00-B. The driver side exterior rearview mirrors are convex (curved glass) and the passenger side mirrors do not include the “Objects in Mirror Are Closer Than They Appear” etched warning. As such, these mirrors fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, “Rear Visibility.” Remedy: Tesla Service will replace the mirrors, free of charge. The infotainment central processing unit (CPU) may overheat during the preparation or process of fast-charging, causing the CPU to lag or restart. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, “Minimum Sound Requirements for Hybrid and Electric Vehicles.” Remedy: Tesla will perform an over-the-air (OTA) software update that will disable the Boombox functionality when the vehicle is in Drive, Neutral and Reverse modes, including Summon and Smart Summon, free of charge. The contact owned a 2021 tesla model s. the contact stated while driving approximately 100 mph, as he took his foot off of the accelerator, with the vehicle brake control set to "aggressive slowdown," as the vehicle slowed the contact looked into the rearview mirror and observed white smoke issuing from the rear of the vehicle. the contact then depressed the brake pedal, and instead of slowing the vehicle accelerated at a dramatic rate and the contact lost control of the vehicle and left the roadway. the contact stated he was not aware of any warning lights being illuminated. the vehicle went into a ditch and continued straight following the ditch. the contact stated that the vehicle had not lost any speed, rather it seemed to be going faster. the vehicle crashed through a fence, drove up a slight berm, and went airborne. the contact's vehicle crashed into the ground, front end first, after flying 130 feet in the air, and began to roll end over end two to three times. the contact and his two passengers were unconscious after impact with the ground, the vehicle had come to a stop and caught on fire. the contact had attempted to exit the vehicle once he regained consciousness, however, he had difficulty since the driver's side door was jammed and his left ankle was dislocated and his right ankle was broken. the contact had managed to crawl out of the vehicle through the driver's side window which had completely shattered. the contact stated that the passenger in the front seat was not seriously injured, although he was transported to a hospital and released that same day. the passenger in the rear was transported and hospitalized with internal bleeding and a broken rib, the contact was not told which rib was broken by the passenger. the vehicle had been engulfed in fire and the contact had rolled into a ditch, a passerby was on the scene and called emergency services. the contact was transported from the scene by a rescue helicopter and flown to a trauma center. at the trauma center, the contact was treated for internal bleeding, the contact stated that 30 percent of his colon was removed, and his spleen had been ruptured. the contact had fractures of both orbits of his eyes, his nose was broken, and he had a severe concussion, with two broken ribs on the right side and one broken rib on the left of his chest. the contact stated that according to a police report and insurance investigation all the air bags had been deployed. the vehicle had burned completely and had to be lifted onto a flatbed truck and was driven to an impound lot. the fire had been so intense that the vehicle's black box was destroyed. the manufacturer had been informed of the failure. the failure mileage was approximately 8,000.

13 Investigations for 2021 TESLA Model S

Smart summon and actually smart summon are features on certain tesla vehicles that allow a user to remotely move the vehicle to the user or to another designated location, through use of a phone app. the oﬃce of defects investigation (odi) has received 1 complaint (voq) alleging that an actually smart summon session resulted in a crash and has reviewed at least 3 media reports of apparently similar crashes. all four incidents involve the subject tesla vehicles operating in actually smart summon failing to detect posts or parked vehicles, resulting in a crash. actually smart summon replaced smart summon and provided users with extended vehicle operating range while expanding vehicle connectivity through phone app live camera feeds and increased user latency experience. odi has received 12 smart summon related voqs alleging crash typology similar to the crashes experienced by users of actually smart summon during sessions with little time for operators to react. odi is aware of multiple crash allegations, involving both smart summon and actually smart summon, where the user had too little reaction time to avoid a crash, either with the available line of sight or releasing the phone app button, which stops the vehicle’s movement. tesla has not reported any smart summon or actually smart summon crashes through the standing general order for crashes involving ads or level 2 adas, which requires reporting of crashes on publicly accessible roads. odi is opening this preliminary evaluation to investigate actually smart summon’s capabilities as they relate to the subject and similar crashes, including the system's operation and ﬁeld performance in fsd equipped vehicles and vehicles included in free trial offers. nhtsa will evaluate the top speed that a vehicle can attain while actually smart summon is engaged, designed-in operating restrictions for usage on public roads, and line of sight requirements. nhtsa expects this preliminary evaluation to include review of remote vehicle control through the phone app at various distances and lines of sight, including app connectivity delays resulting in increased stopping distance and the ability to utilize actually smart summon in roadway environments or operating conditions for which the current version of the system is neither intended nor designed. to review the odi reports cited in the opening resume odi report identiﬁcation number document, go to nhtsa.gov.

The office of defects investigation (odi) upgraded pe21020 to ea22002 on june 8, 2022, to extend work and deepen the pe21020 crash analysis, to supplement that analysis with additional data, and to perform vehicle evaluations to understand how tesla’s autopilot system may exacerbate human factors or behavioral safety risks by undermining the effectiveness of the driver’s supervision. to support this work, odi collected additional crash information and assessed vehicle control authority, driver engagement technologies, and related human factors considerations associated with partial automation via analysis of peer vehicle data and hands-on vehicle evaluation, assessments from nhtsa human factors subject matter experts, and reviews of related publications dedicated to partial driving automation. autopilot is the simultaneous use of the features that tesla calls traffic-aware cruise control (tacc) and autosteer. tacc is a type of adaptive cruise control that, like traditional cruise control, maintains a set speed but also slows or accelerates as necessary to maintain the vehicle’s following distance from a vehicle in front. as designed, autosteer detects lane markings and the presence of other nearby vehicles and objects to keep the vehicle in its driving lane. autopilot is characterized by tesla as an sae level 2 (“l2 system”) partial driving automation system that provides driver assistance through steering, propulsion, and braking within a specified driving environment under direct supervision of the driver. l2 systems should be designed to support the driver’s need to monitor the system in response to the constantly changing driving environment and, if necessary, take over the dynamic driving task. to ensure sufficient driver engagement, vehicles with l2 systems should employ driver engagement systems and usage controls that are appropriate and sufficient for the l2 system design and driver expectations. odi completed an analysis of 956 crashes reported up to august 30, 2023. in approximately half (489) of those crashes, odi found: 1.) that there was insufficient data to make an assessment; 2.) the other vehicle was at fault; 3.) autopilot was found to not be in use; or 4.) the crash was otherwise unrelated to ea22002. of the remaining 467 crashes, odi identified trends resulting in three categories: collisions in which the frontal plane of the tesla struck another vehicle or obstacle with adequate time for an attentive driver to respond to avoid or mitigate the crash (211), roadway departures where autosteerwas inadvertently disengaged by the driver’s inputs (111), and roadway departures in low traction conditions such as wet roadways (145). odi observed this pattern across all tesla models and hardware versions. crash and human factors assessment showed that autopilot controls did not sufficiently ensure driver attention and appropriate use. at the same time, peer analysis and vehicle evaluations established that autopilot invited greater driver confidence via its higher control authority and ease of engagement. this mismatch of weak usage controls and high control authority was evident in these crash categories, which included indications of driver disengagement from the driving task. this mismatch was also evident in roadway departures when the system was engaged in low traction conditions outside of tesla’s recommendations.additional information regarding nhtsa’s crash analysis is available in the ea22002 file. odi reviewed these findings with tesla during several conversations in quarter 4 of 2023. on december 12, 2023, tesla filed a defect information report (dir) (recall 23v838) applicable to all tesla models produced and equipped with any version of its autopilot system. tesla’s dir described the functionality of this system, stated that the prominence and scope of the system’s controls may be insufficient to prevent driver misuse, and described a remedy to improve the effectiveness of driver warnings and to reduce m
