These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the front air bags, these inflators may rupture due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling. Remedy: Toyota will notify the Toyota and Lexus owners. General Motors will contact the Pontiac owners. Depending on the model, dealers will replace the passenger frontal air bag inflator or the air bag assembly, free of charge. The recall began on December 6, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota’s numbers for interim notification for this campaign are G1P (Toyota Zone A), G1R (Toyota Zone B), G2G (Lexus Zone A), and G2H (Lexus Zone B). Additionally, unless included in “Zone A” above, Toyota is recalling certain model year 2008 Scion xB and Lexus IS F, 2007-2008 Toyota Yaris, 2006-2008 Lexus IS250 and Lexus IS350 and 2007-2008 Lexus ES350 vehicles, originally sold, or ever registered, in Arizona, Arkansas, Delaware, District of Columbia, Illinois, Indiana, Kansas, Kentucky, Maryland, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, Tennessee, Virginia, and West Virginia, or “Zone B.”

Vehicles of these model years and models that were not originally sold or ever registered in either Zones A or B are not subject to this safety recall.

These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the front air bags, these inflators may rupture due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.

Remedy:

Toyota will notify the Toyota and Lexus owners. General Motors will contact the Pontiac owners. Depending on the model, dealers will replace the passenger frontal air bag inflator or the air bag assembly, free of charge. The recall began on December 6, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota’s numbers for interim notification for this campaign are G1P (Toyota Zone A), G1R (Toyota Zone B), G2G (Lexus Zone A), and G2H (Lexus Zone B). Letter One: December 11, 2009 Toyota Motor Sales USA, Inc. Department H200 19001 S. Western Avenue Torrance, CA 90509 Reference #:0912104255 This is a follow-up to my phone call of December 3, 2009. I was involved in an accident with my Toyota Matrix S 2009 on October 9, 2009 at about 3:30 PM. The accident took place at the Toyota of Morristown, N. J. dealership. I was slowly backing out of a parking space. I stopped because a saw a car that looked like it wanted to pass behind me. After that vehicle passed me I backed out of the parking space. As I proceeded forward, I thought there was enough room for my front passenger wheel to clear the small brickwork separating the parking lot from the dealer’s incoming and outgoing traffic. The wheel did not clear the brickwork. I put my vehicle in reverse to give me more room to go around this brickwork. However, when I shifted the car into reverse it accelerated. The brakes did not work. My car backed into a another vehicle, turned 180 degrees around, hitting a wooden fence. My seat belt was on and the air bags did not deploy. I removed the key from the ignition, got out of the Matrix, removed my pet then closed the car door. Many of the sales personnel came out of the showroom to see what had happened. I walked to the back of the car and sat on the curb. One of the salesman opened my driver’s side door and another salesman took out the all-weather mat that I had on the floor. They were asking why I had this mat in my car. The salesmen said I shouldn’t have this mat from another Matrix in my car. I told them that when I purchased my Matrix, I went to the Toyota Parts Department next door. The salesman there advised me not to purchase new mats for my Matrix. He said the old mats were fine and there is no need to waste money. He also said to make sure the mat does not go under the pedals. A service person from the dealership came over to the accident scene and escorted me inside the service area. I sat down to calm myself and checked my pet to make sure she was alright. After a few minutes the service person came back and told me the police had arrived. I went back to my car. The officer asked for an explanation of what had happened and to give him my driver’s license and insurance card. He asked for the keys and went inside the Matrix and started the car. It was at this point I realized the drivers’ side all-weather mat was gone. I was still shaken up by the accident and didn’t think to say something. The officer then went back to his patrol car. I opened the back door of the car and sat there until the officer came over to me. He gave me a ticket for careless driving. He also wanted to tow my car. One of the salesmen that remained by my car told the officer not to tow the car, it could remain at the dealership. The police officer threatened to impound my car, but I noticed that a salesman went to speak to the officer. The officer then called me back over to say he changed his mind and would not impound my car and it could remain at the dealership. I went back to the service area where I rented another car and I returned to the Toyota dealership about 7:00 PM. I wanted to get my personal items from my car. The Matrix had been moved to a parking space close to the service area. I opened the trunk. There was the all-weather mat. I removed my all personal items. I then went into the service area. A service person pulled me aside and asked me who removed my mat from the drivers’ side? I said it was one of the salesmen, but since there were so many that surrounded me and the car, I didn’t know which one of the salespeople took the mat. The service person then told me that the reason I could not brake my car was due to the all-weather mat. It was curled under which, prevented the brake pedal from being depressed and the salesman was probably trying to protect Toyota by removing the mat and placing it in my trunk. The service person also said that if the mat had not been taken out, the police officer would have seen the mat curled under the pedal and would not have given me a ticket. This accident could have been prevented if: • I received a re-call notice concerning Matrix’s and all-weather mats and uncontrolled acceleration. Friends of mine who own Camry’s and Avalon’s received letters from Toyota; and/or • the salesman at the Toyota parts department had sold me the correct mats for my Matrix. I would not have received a ticket if: • on the day of the accident, the Toyota salesman never removed my all-weather mat from my car. The salesmen should not have touched the car. This was the scene of an accident. Besides having to purchase another car, which I did from the same Toyota dealership, I incurred the cost of a new vehicle, a $500 deductible on my policy and I had to hire an attorney to represent me in court due to the ticket. Western Avenue Torrance, CA 90509 ATTN: Legal Department Reference #:0912104255 This is a follow-up to my phone conversation of December 17, 2009 with “John” at Toyota Corporate Headquarters. On December 17, “John” responded by phone to my letter (see attached) that Toyota cannot take any responsibility for the all-weather mat contributing to my accident on October 9, 2009. He said there is no “recall” on Toyota vehicles and Toyota has to monitor complaints that are “coming out of the woodwork”. My accident is not a complaint coming out of the woodwork. I am very grateful that the sudden acceleration of my Matrix did not harm or kill anyone. I was able to walk away from this accident with my pet, even though my vehicle was totaled. I asked “John” to put what he said in writing – that the Toyota Corporation has no responsibility. He could not, and suggested I write to Toyota’s Legal Department. Please forward a letter to my attention stating that the sudden accelerations of Toyota vehicles and the contributing factor of the all-weather mats is not a recall and the Matrix model does not have this very dangerous problem.
