2016-06-02	SEATS:FRONT ASSEMBLY:SEAT HEATER/COOLER	Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed. Remedy: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. 2016-02-05	AIR BAGS:SIDE/WINDOW	Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2003-2006 Toyota Land Cruiser and Lexus LX470, 2005-2006 Toyota Tundra and Sequoia, 2004-2006 Toyota 4Runner and Lexus GX470 vehicles equipped with side curtain-shield-air bags. The affected vehicles have an air bag control module that may be improperly programmed, causing the side curtain-shield-air bags to deploy inadvertently. Remedy: Toyota will notify owners, and dealers will replace the air bag control module with one that has improved programming, free of charge. The recall began on December 1, 2016. Owners may contact Toyota customer service at 1-888-271-9371. Toyota’s number for this recall is G0C and GLB.
2011-02-24	VEHICLE SPEED CONTROL:ACCELERATOR PEDAL	TOYOTA IS RECALLING CERTAIN MODEL YEAR 2008 THROUGH 2011 LEXUS LX570, 2003 THROUGH 2009 TOYOTA 4RUNNER, AND 2006 THROUGH 2010 RAV4 VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER’S FLOOR MAT. Remedy: THE REMEDY IS NOW AVAILABLE. THE SAFETY RECALL BEGAN EARLY APRIL 2011. THE REMEDY WILL INVOLVE MODIFICATION OR REPLACEMENT OF THE ACCELERATOR PEDAL AND REPLACEMENT OF ANY TOYOTA ALL-WEATHER FLOOR MAT. I was coming into Dewitt Arkansas on Highway 165 on 12/13/09 at about 12:30pm with my cruise control on at about 60mph. I tapped the brake to disengage the cruise control and slowed down to make a right hand turn into an Exxon Mini Mart. My 2006 Toyota 4runner suddenly accelerated as I attempted to stop near the front door. As I applied pressure to the brakes I heard the anti lock brakes clicking and suddenly the 4runner lunged forward striking a large pole that was in front of me. The rear wheels were spinning wildly with the 4runner against the pole and the back end was bouncing up and down. The 4runner was bouncing to the side and I slammed the shift lever into park. The power to the back wheels stopped. I sat back and took my foot off the brake pedal, then I noticed that the engine was still running wide open throttle. I reached up and turned the key off. I looked at the floor mat and it was in its normal location with no contact with the gas pedal. The two hooks were attached holding the floor mat in place. There was no way the floor mat could have been involved. Two employees came out and asked if I was okay, I told them I was okay, but scared to death. I told them I heard about Toyota’s having a sudden acceleration problem, but I never thought I would be involved in one. I had to walk around a while to calm down. I got the employee’s names and phone numbers in case I needed them. I went back outside and started the 4runner up again and it ran normal. I drove it to my Dad’s and left it there. I called the Toyota dealer the next day and had them come pick it up. The dealer had a local technician (Doug) check it out. They could not reproduce the event and reported no trouble codes. They asked that I come and get the 4runner. I told them this was unacceptable. They had a regional technical specialist (Todd Troclaise) fly in from Houston to check the 4runner out. The official letter I got from Gulf States Toyota representative Cathy McWilliams states; " We are writing to advise our investigation has been completed in regards to your concern of whether your 4runner experienced sudden acceleration during the recent accident. Our Technical Specialist inspected your vehicle at North Point Toyota on January 4, 2010 for the concern of sudden acceleration. It was noted that during the inspection, the driver’s floor mat was in place and properly secured. There were no codes stored in the computer to indicate any product concern or failure. Our Technical Specialist noted that aftermarket pedal covers were installed on the brake and accelerator pedals that increase the length pedals, which could have contributed to the accident described. Our Technical Specialist was unable to find any product defects that could have contributed to the circumstances you described. As no factory defects were found, we are unable to offer any further assistance. We than you for bringing this matter to Toyota’s attention and allowing us the opportunity to investigate your concern." I was told by the local dealer (North Point Toyota) that there were no safety hazards with my 4runner and I needed to come get my it. I told them there was a safety hazard and I would not drive the 4runner. I told them they could drive it to my house and park it in my driveway if they wanted it off their lot, which they did. The 4runner has sat in my drive way since. I am still making payments on my 4runner and insurance payments for a vehicle I won’t drive due to safety concerns. I have filed a complaint with the Arkansas Attorney General’s office of Consumer Protection and the National Highway Traffic Safety Administration. I got a confirmation from NHTSA that they received my complaint (#10299572). The AR AG complaint number is 79222. The AG’s office recently sent my a copy of the offical letter from Toyota and asked for a response if I did not agree. Out of frustration I filed a class action lawsuit on 2/19/10. I recently read an article that recommended that I report this information to your website as well. No repairs have been done to date. The aftermarket pedals described in the letter are OBX Sport Pedals that are a Toyota dealer installed option that were put on before I took possession of the vehicle. There is a Toyota part number for them. I don’t believe they had anything to do with my incident.

3 Investigations for 2006 TOYOTA 4Runner

On february 16, 2010, nhtsa's office of defects investigation (odi) opened rq10-003 to determine whether the scope of prior toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, toyota recalls 07e-082, 09v-388, 10v-017, and 10v-023.nhtsa also requested information regarding potential electronic causes of unintended acceleration in toyota and lexus vehicles.i. pedal interference from floor mats, carpet covers and plastic pads in carpets.during the rq10-003 investigation, odi reviewed a large volume of documents to assess whether additional vehicles should be recalled.following the agency's analysis, nhtsa requested that toyota recall additional vehicles.toyota complied with the agency's request.the details of these recalls are set forth more fully in toyota's reports to nhtsa pursuant to 49 cfr part 573.these recalls are as follows:recall 11v-112: (1) model year (my) 2004-2006 toyota highlander and highlander hybrid; and, (2) my 2004-2007 lexus rx330,rx350, and rx400h (hybrid model). the total estimated population under this recall is 769,379 vehicles.this recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). recall 11v-113: (1) my 2003-2009 through 2009 toyota 4runner; (2) my 2006-2010 toyota rav4; and, (3) my 2008-2011 lexus lx570.the total estimated population under this recall is 1,381,000 vehicles.this recall supplements recall 09v-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.recall 11v-115: (1) my 2006-2007 lexus gs300 (all wheel drive vehicles); and, (2) my 2006-2007 lexus gs350 (all wheel drive vehicles). the total estimated population under this recall is 19,647 vehicles.this recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.ii. potential electronic causes of unintended acceleration.after nhtsa opened rq10-003, nhtsa launched a ten-month study of potential electronic causes of unintended acceleration in toyota vehicles.nhtsa launched the study in the spring of 2010 in light of concerns aired in congressional hearings.nhtsa enlisted engineers at the national aeronautics and space administration (nasa) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in toyota vehicles.that study has concluded.two reports are associated with the study and are available on nhtsa's website.nasa's report is entitled "technical support to the national highway traffic safety administration (nhtsa) on the reported toyota motor corporation (tmc) unintended acceleration (ua) investigation," nesc assessment no. ti-10-00618 (jan. 18, 2011).nhtsa's report is entitled "technical assessment of toyota electronic throttle control (etc) systems" (feb. 2011).both reports should be read in conjunction with each other. as stated in its report, nasa did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. nhtsa did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by toyota's recalls. this rq is closed.
