Defect: Front suspension coil springs have insufficient corrosion protection, and may be susceptible to corrosion which can cause spring failure. Broken spring could puncture front tire and result in crash.
Remedy:  Correct by replacing both front springs with improved springs.

Defect: Wiring harness for occupant detection system (ODS) and occupant position detection system (OPDS) is routed underneath carpet on driver’s side floorboard. Salt from snow on driver’s shoes will melt and may penetrate carpet and leak into wire harness. If harness is exposed to salt brine, corrosion may occur and wire may break causing SRS indicator light to illuminate. Failure of ODS and OPDS may not detect presence of child seat or out-of-position passenger and deployment of both front seat passenger’s frontal and side airbags will not be suppressed. Deploying front passenger airbag or front passenger’s side airbag will increase risk of injury to small or out-of-position occupants.
Remedy:  Correct by inspecting and replacing wire harness.

Defect: In states that use corrosive materials for deicing, driver’s footwear may bring in materials that saturate vehicle’s carpeting, which is in contact with vehicle’s engine electronic control unit (ecu). Ecu case may experience rust or corrosion which may cause engine to stall, resulting in crash.
Remedy:  Correct by inspecting and replacing any damaged ecu and by installing waterproof cover onto ecu.

Defect: bonded disc brake linings can separate from shoes due to bonding deficiency which allows corrosive degradation of bonded attachment. Separation of disc linings results in an increase in stopping distance.
Remedy:  Correct by replacing front and rear brake linings on vehicles with 15 inch brakes, but replace only rear brake linings on vehicles with 14 inch brakes because front linings on these vehicles are riveted.

Defect: front crash sensors may crack under certain environmental conditions and allow water to enter into sensor. This can cause sensor to become inoperative and illuminate airbag warning light. Defect: Front disc brake rotors may fracture or separate near inner edges of brake discs, resulting in reduced braking effectiveness, higher than normal pedal efforts, loud grinding noises, and moderate pulls when brakes are applied. Reduced braking effectiveness many increase minimum stopping distances, possibly resulting in an accident.
Remedy:  Correct by replacing front brake rotors with full cast front brake rotors.

Defect: body mounts at rear corners of subframe, which supports engine and transmission, may become detached due to corrosion of subframe mount retaining plate. This would allow rear corners of subframe to drop. If one of rear corners drops, only minor indications of malfunction such as clunking noise or slight change in steering wheel alignment are noticeable. However, if both rear corners of subframe drop, steering becomes very difficult, which could adversely affect vehicle control and possibly result in an accident.
Remedy:  Install corrosion resistant reinforcing plate and new bolt on both rear subframe mounts.

Vehicles:  1987-94 Ford B600 and B700 medium duty school bus chassis with hydraulic brakes, tilt hoods, 5.9L, 6.6L, or 7.6L diesel engines or 6.1L or 7.0L gasoline engines made from September 1986 through March 1994.
Population: 3,660 chassis sold or currently registered in Connecticut, Illinois, Indiana, Maine, Massachusetts, Michigan, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, and Wisconsin.
Defect: battery power junction block mounted on right fender apron, which serves as an electrical connection point for several engine compartment systems, is susceptible to road splash. terminals at junction block can experience corrosion and can fracture, causing loss of electrical power and engine shutdown. Engine shutdown, loss of power steering assist, or loss of hydraulic brake boost can occur which could result in loss of vehicle control and crash.
Remedy:  Correct by replacing junction box, main power terminal, starter or starter relay, and terminals at existing junction block. wiring of hydraulic brake circuit will also be modified by bypassing junction block. new terminals will be made from an alloy material more resistant to salt corrosion.

NHTSA Recall No. 95V-231.001/Ford Recall No. 95S29
Vehicles:  1993-94 Ford Ranger with 2.3L engine made from August 1992, through December 1994.
Population: 26,000 pickup trucks sold or currently registered in Alaska, Montana, North Dakota, Minnesota, Wisconsin, Michigan’s Upper Peninsula, New York, Maine, Vermont, and New Hampshire.
Defect: During extreme cold, ice can form in throttle body of engine causing throttle plate to remain in highway cruising position after accelerator pedal is released, or if cruise control was active, after cruise control was deactivated. This can cause extended stopping distances, increasing risk of crash.
Remedy:  Correct by replacing air intake duct and valve assembly with revised units.
 
NHTSA Recall No. 96V-024.001/Ford Recall No. 96S35
Vehicles:  1991-94 Ford Explorer, Ranger made from March 1991 through November 1993.
Population: 108,000 sport utility vehicles and pickup trucks originally sold or currently registered in Southern California counties of Los Angeles, Orange, San Bernardino, San Diego, Riverside, Ventura, Santa Barbara, San Luis Obispo, Imperial, and Kern.
Defect: studs that attach brake master cylinder to power brake vacuum booster assembly can develop stress corrosion cracking after being operated for an extended period of time in Southern California location. If crack occurs, it could result in one or both studs fracturing. master cylinder could then separate from vacuum booster, when brakes are applied, preventing service brakes from activating increasing risk of vehicle accident.
Remedy:  Correct by replacing power brake vacuum booster assembly.

Vehicles:  1997 Ford F150 made from November 1995 through July 1996.
Population: 22,000 pickup trucks sold or currently registered in Alaska, Montana, North Dakota, Minnesota, Wisconsin, Michigan (Upper Peninsula), New York, Maine, Vermont, and New Hampshire.
Defect: Operation at highway speeds during extreme cold, northern climate winter conditions can result in ice forming in throttle body of engine. This ice can cause throttle plate to remain in highway cruising position after accelerator is released or speed control is deactivated. Under such conditions, vehicle stopping distances would be increased possibly resulting in crash.
Remedy:  Correct by installing foam-insulating sleeve on line from PCV to hose fitting on throttle plate.

NHTSA Recall No. 97V-019/Ford Recall No. 97S66
Vehicles:  1992-94 Ford Tempo, Mercury Topaz with 3.0L engines; 1994 Ford Tempo, Mercury Topaz with 2.3L engines; 1992-95 Ford Taurus, Mercury Sable with 3.8L engines; 1994-95 Ford Taurus, Mercury Sable with 3.0L engines; and 1992-94 Lincoln Continental with 3.8L engines made from July 1991 through November 1994.
Population: 75,200 passenger vehicles sold or currently registered in Alaska, Iowa, Minnesota, Nebraska, North Dakota, and South Dakota.
Defect: During high winds, heavy, blowing, and drifting snow, and low temperatures, engine cooling fan can become blocked or frozen with snow. fan motor may not rotate and could overheat. This can cause smoke or flames from fan and/or shroud, fan electrical wiring, or fan motor, resulting in vehicle fire.
Remedy:  Correct by installing an electrical jumper harness containing an automatic resetting circuit breaker to protect fan motor low speed circuitry from overheating.

NHTSA Recall No. 97V-025/Ford Recall No. 97S65
Vehicles:  1991-94 Ford Taurus, Mercury Sable, Lincoln Continental with 3.8L engines made from August 1990 through November 1994.
Population: 212,700 passenger vehicles sold or currently registered in Alaska, Colorado, Iowa, Idaho, Illinois, Indiana, Kansas, Massachusetts, Maine, Michigan, Minnesota, Missouri, Montana, North Dakota, Nebraska, New Hampshire, New York, Ohio, Pennsylvania, South Dakota, Vermont, Wisconsin, and Wyoming.
Defect: Water can accumulate within speed control cable conduit. If enough water accumulates, it could freeze within low area of cable routing when exposed to long term cold soak at temperatures at least several degrees below freezing. Driver control of vehicle speed using speed control system or accelerator control system would be diminished increasing risk of crash.
Remedy:  Correct by add boot to speed control cable.

NHTSA Recall No. 97V-074/Ford Recall No. 97S67
Vehicles:  1996-97 Ford Explorer, Mercury Mountaineer made from February 1996 through February 1997.
Population: 23,000 sport utility vehicles sold or currently registered in Alaska, Idaho, Iowa, Maine, Michigan (Upper Peninsula only), Minnesota, Montana, Nebraska, New Hampshire, New York, North Dakota, South Dakota, Vermont, Wisconsin, and Wyoming.
Defect: These vehicles, when operated for extended period of time at highway speeds and temperatures below -20 degrees Fahrenheit, do not return to idle due to ice in throttle body. Although vehicle can be controlled by either applying service brakes or shifting transmission to neutral, increased brake pedal efforts or some increase in stopping distance can result.
Remedy:  Correct by installing revised PCV system with water-heated vacuum source.

NHTSA Recall No. 97V-144/Ford Recall No. 97S79
Vehicles:  1995 Ford Escort, Mercury Tracer with 1.9L engines made from April through October 1995.
Population: 64,000 vehicles built at Hermosillo or Wayne Assembly Plants, and originally sold or currently registered in Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, Nevada, OklahoMassachusetts, South Carolina, and Texas.
Defect: Cracks can develop in fuel tank near heat shield attachment resulting in fuel leakage if cracks go through fuel tank wall. Fuel leakage in presence of an ignition source can result in vehicle fire.
Remedy:  Correct by remove heat shield attachments. This will not affect heat shield location or function, as attachments are intended to keep shield in place until fuel tank is installed in vehicle. After installation, fuel tank straps retain heat shield.

Defect: Cracks can develop in fuel tank near heat shield attachment resulting in fuel leakage if cracks go through fuel tank wall. Fuel leakage in presence of an ignition source can result in vehicle fire.
Remedy:  Correct by remove heat shield attachments. This will not affect heat shield location or function, as the
attachments are intended to keep shield in place until fuel tank is installed in vehicle. After installation, fuel tank straps retain heat shield.

Vehicles:  1992-93 Ford Thunderbird, Mercury Cougar made from August 1991 through September 1993.

Defect: Movement of fuel lines causes nylon fuel line jumper to chafe against floor pan. This wear can cause pin hole in fuel line resulting in fuel leakage. This fuel leakage results in dampness and fuel odor, and if an ignition source was present, fire could result.
Remedy:  Correct by installing plastic convolute covering around fuel lines. If lines show leak, lines will be replaced first.

Defect: throttle is unable to return to idle due to ice forming in throttle body when temperature ranges from -10 to -40 degrees Fahrenheit. Increased brake pedal efforts or some increase in stopping distance could result.
Remedy:  Correct by installing revised PCV system on these vehicles.

Defect: front coil springs can fracture as result of corrosion in combination with small cracks in springs. front tire could deflate due to broken front coil spring contacting tire, increasing risk of vehicle crash.
Remedy:  Correct by installing spring catcher bracket that will prevent fractured spring from contacting tire.

Remedy:  Correct by replacing fuel tank, regardless of mileage, through March 31, 2002. This coverage will only be structural cracks at top of tank and will not include other damage caused by crash or road debris.

Defect: rear lower sub frame mount plate nut can experience stress corrosion cracking if subjected to long-term exposure to road salts which are used extensively in above listed states. Detachment of body mounts at rear corners of sub frame, which supports engine and transmission, would allow rear corners of sub frame to drop. If both rear corners of sub frame drop, steering could become suddenly very difficult, adversely affecting vehicle control and possibly result in crash.
Remedy:  Correct by installing sub frame rear mount bolts, reinforcement plates, and plate nuts.

Defect: These mini vans can develop cracks in fuel tank in forward strap area of standard 20-gallon tank due to combination of factors that are present in certain very hot areas of country. cracks could result in fuel leakage if they propagate through wall of tank. Fuel leakage, in present of an ignition source, could result in fire.
Remedy:  Correct by installing brace assembly at strap bolt hole location and replace tank strap with revised, longer strap. Correct by also inspect tanks for leaks. If leak exists and is result of crack in fuel tank, tank will be replaced in addition to installation of spacer and longer strap.

Defect: During assembly, some brake lines were installed in twisted position causing contact with dash panel insulator above catalytic converter. This could abrade brake line corrosion protection, and also salt or water absorbed in dash panel insulator would promote corrosion. This corrosion could ultimately lead to leak in brake line. Continued loss of brake fluid will result in brake light activation. Extended stopping distance could also result if significant fluid loss occurs, increasing risk of crash.
Remedy:  Correct by inspect brake line for corrosion and clearance conditions. brake line that is not corroded will be repositioned. brake line that is corroded or leaking will be replaced with nylon coated brake line and secured using three retaining clips to assure clearance with insulator.

Remedy:  Correct by installing protective spring shields. In addition, Ford is extending warranty coverage of front coil springs in all other states, for fracture only, up to 10 years of service or 150,000 miles, whichever occurs first. If vehicle has already accumulated more than 150,000 miles, this coverage will last until July 31, 2005. If either front coil spring fractures, Correct by replacing both front springs at no charge to owner. an interim notice was sent to owners on August 2, 2004. Parts are now available for 2000-01 MY vehicles and owners were sent letter on January 10, 2005, advising them to bring in their vehicles for repair Parts are expected to be available for 1999 MY vehicles by end of February 2005 and owners of these vehicles will be notified then.

Remedy:  Correct by replacing fuel tank straps with straps that have increased corrosion protection. If replacement straps are not available, correct by installing cable support under strap as interim repair or steel reinforcement over existing strap as permanent repair.

Remedy:  Correct by installing shielding insulator under junction block.

Remedy:  Correct by replacing front brake rotors with new rotors with corrosion protection coating. However, if replacement of brake linings is necessary, it will be done at owner’s expense since brake linings are normal maintenance items.

Defect: While driving at steady throttle opening for an extended period of time during extremely cold ambient temperatures (below approximately -5 degrees Fahrenheit with high humidity), moisture in combustion chamber blowby gases can freeze and block positive crankcase ventilation (PCV) valve and hose. If PCV hose freezes, combustion chamber gases are forced through breather hose into air intake hose and then into throttle body. Moisture in combustion chamber blowby gases may freeze inside throttle body bore and throttle body idle speed control circuit resulting in throttle valve not closing fully and higher than normal idle speed when accelerator pedal is released.
Remedy:  Correct by installing new, relocated PCV hose and will additionally install heated throttle body on 2000-2001 Accents with 1.5-liter engines that were produced through January 31, 2001.

Remedy:  Correct by inspecting rear trailing arms for internal corrosion damage. If specified levels of corrosion damage are found, rear trailing arms will be replaced. Rear trailing arms that do not require replacement will have drainage holes added and will be treated with rust-proofing material to arrest corrosion process. Remedy:  Correct by replacing power brake vacuum booster assembly.

In addition to performing recall campaign, Mazda is extending warranty coverage on front coil springs for lift of vehicle. If owners have already paid for repair or replacement of these front coil springs, due to road salt damage, Mazda will reimburse these owners for reasonable expenses.

As result, when door is opened door latch pawl may remain in open position resulting in door not being able to be closed and latched. If vehicle is operated with an open door it may allow an unbelted occupant to be ejected from vehicle which could result in injury or death.
Remedy:  Correct by replacing sliding door latches with modified latches.

This issue could result in non-deployment of driver and passenger front airbags in crash, and result in personal injury.
Remedy:  Correct by replacing front czs with redesigned sensor. Owners in other states will receive extended warranty coverage for sensor to 10 years. owners will be notified of warranty extension by mail and will receive sticker to place in their warranty booklet explaining extended warranty coverage.

This causes an electrical short inside harness, and can result in vehicle fire.

Remedy:  Correct by repairing vacuum port. If Echo owner believes that there is possibility that vehicle will be operated in these states during winter or in conditions described, vehicle should be serviced for this recall.

Defect: Excessive corrosion of rear cross member may cause separation of spare tire stowed under truck bed which would present road hazard that could cause crash. Separation of spare tire and diminished brake capacity could increase risk of crash. Toyota is asking owners of affected vehicles to remove spare tire from under vehicle until remedy can be obtained.
Remedy:  Correct by inspecting rear cross member and related components, including brake lines, and replace them if necessary. If cross member does not need to be replaced, it will be sprayed with corrosion protection compound. services will be performed, however multiple visits to dealer may be required.

Remedy:  Correct by inspecting for cable corrosion and applying anti-rust agent if none is found. Correct by cutting and recrimping cable if rust is found and applying anti-rust agent. On 2004-10 models, in addition correct by installing water splash protector.

Defect: While driving during extremely low temperatures, throttle may freeze in an open position, due to ice accumulating in throttle body. vehicle may fail to loose speed when driver lifts his or her foot from gas pedal which could result in loss of vehicle control and crash without warning.
Remedy:  Correct by installing replacement throttle-body kit in vehicles operated in above states. 11,800 owners in other states will be notified in service campaign (No. JG) and may obtain free throttle kit upon request.

NHTSA Recall No. 04V-368/Volvo Recall No. 139
Vehicles:  1994-97 Volvo 850 made from July 1993 through December 1996.
Population: 105,000 passenger cars originally sold or currently registered in hot weather Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, Nevada, New Mexico, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, and Utah.
Defect: After prolonged exposure to heat, fuel tank may seep or leak fuel at points where heat shield is attached. Fuel leakage could result in fire.
Remedy:  Correct by inspect fuel tank and replace it if any evidence of fuel seepage or leakage is found. All vehicles will receive new reinforcement bushings at heat shield attachment points to prevent future leaks. 121,500 passenger cars in other 33 states and District of Columbia will be covered by service campaign.

NHTSA Recall No. 08V-033/Volvo Recall No. 190.
Vehicles:  2004-06 Volvo S40, V50 made from November 2002 through August 2005.
Population: 23,000 vehicles sold in or currently registered in Connecticut, Delaware, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, Wisconsin, and District of Columbia.
Defect: Reoccurring, long-term exposure to environmental conditions such as salty-wet conditions can corrode pump electronic module (pem) internally. This can cause faulty signal to fuel pump, resulting in low or no fuel pressure to engine. If this occurs, check engine light may illuminate. In some cases, vehicle may fail to start or may stall, resulting in crash.
Remedy: Correct by replacing pem and installing it in new location to prevent corrosion.

Vehicles:  2001-04 Volvo S60, V70, XC70 and 2001-05 S80 and 2003-05 XC90 made from October 2000 through January 2005.
Population: 158,733 vehicles sold in or currently registered in  Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Kentucky, Louisiana, Mississippi, Nevada, New Mexico, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, Utah, Virginia, and U.S. Territories.
Defect: In hot climate and temperature cycling, and in combination with varying fuel quality, fuel pump can develop cracks in fuel pump flange which can result in fuel and fire
Remedy:  Correct by inspecting fuel pump and replacing as necessary.
